why you are applying for this scholarship:

Thank you for your interest in the Otho Davis Scholarship.
We will confirm your application receipt within two weeks.

We will award one student a scholarship in the amount of $5,000.  The qualifications for an applicant are as follows:

Participation in a graduate or undergraduate program in the area of a) sports medicine, b) physical training, or c) sports marketing.

The applicant must also exhibit significant financial need.  This determination must be addressed or the application will not be considered.  A letter from the financial aid office will usually suffice.

      After filling out the above form, if you are considered to be a good candidate, you will be asked to provide the following

      An introductory letter written by the candidate, introducing him/herself and explaining his/her current curriculum and professional goals.  The letter should also include a brief background and provide the trustees with a flavor of the individual and his/her experiences and long-term goals.

        A resume reflecting the standard education and work history, but we would like them to also include any extra curricular activities and service projects in which they have participated.  Service work is very important.

          A letter of recommendation from the Dean of the school in which the student is enrolled (please include GPA).

            The scholarship recipient will be acknowledged at our annual Awards Dinner.
